Voriconazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the gist: your body relies on a liver enzyme called CYP2C19 to break down voriconazole. Lab research hints that lentinan might rev up that enzyme. If it does, your system could clear voriconazole faster than usual, leaving lower levels in your blood. Voriconazole isn't a prodrug, so less drug simply means less antifungal punch, and that matters a lot when you're fighting a serious infection like aspergillosis or candidemia.

The important caveat: this is only test-tube and animal data, and it has never actually been seen in people. So I wouldn't panic. Still, since letting a fungal infection slip is risky, please loop in your doctor or pharmacist before adding lentinan rather than stopping or changing anything on your own.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the situation with these two. Voriconazole is cleared from your body partly by a liver enzyme called CYP3A4. Lab and animal research hints that lentinan may speed up that enzyme. If that happened in people, your body could clear voriconazole a little faster, which might lower its levels and make it less effective against a serious fungal infection. That matters, because voriconazole already has very swingy, hard-to-predict blood levels.

I want to be honest about the strength here: this is only test-tube and animal data, and it has not been seen in real patients. So please don't panic. Still, since voriconazole is treating something serious, it's worth a quick chat with your doctor or pharmacist before adding lentinan rather than guessing on your own.

Evidence for oral supplements is limited, and quality varies widely between products. About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
