Lentinan and Warfarin Sodium

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Lentinan interacts with whole categories of medication. Warfarin Sodium falls into 3 of those categories, so it can interact with Lentinan in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Warfarin Sodium

Warfarin Sodium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for warfarin. Lab tests hint that lentinan might speed up an enzyme called CYP1A2, which helps clear certain medicines. In theory, faster clearance could lower the level of a drug in your body. For warfarin, that could mean a little less blood-thinning effect, so your INR (the blood test that guides your dose) might drift lower and your clot protection could weaken slightly.

I want to be honest, though: this is only seen in laboratory and animal work, not in real patients, and warfarin's main pathways involve other enzymes (mostly CYP2C9), not just CYP1A2. Still, warfarin is a narrow-margin drug where small shifts matter. If you're considering lentinan, please loop in your pharmacist or doctor first and keep up your regular INR checks.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

2 of 3

Warfarin Sodium also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the gist for warfarin. Lab studies hint that lentinan might speed up an enzyme called CYP2C19, which helps clear warfarin. If that enzyme works faster, your body could break down warfarin a little quicker, which might make it slightly less effective at thinning your blood. In theory, that could raise your risk of a clot forming.

I want to be honest, though: this has only been seen in test tube and animal research, never in real people, so it's far from proven. Warfarin is still a tricky drug where even small swings show up on your INR blood test. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first, and keep up with your regular INR checks.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Warfarin Sodium also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for warfarin. Some lab research hints that lentinan might speed up a liver enzyme called CYP3A4, which is one of the pathways that helps clear warfarin. If that enzyme runs faster, your body could break warfarin down a bit quicker, which in theory might make it work a little less. For a blood thinner, that could mean your INR drifts lower and your protection against clots weakens.

That said, this is only a theoretical signal from test tubes and animals, never actually seen in people. Warfarin is also a narrow-window drug, so even small swings matter and your INR is your safety net. If you and your doctor decide to add lentinan, just ask them to keep an eye on your INR rather than changing anything on your own.

About Lentinan

What is Lentinan?

Lentinan
  • Immune support
  • Used alongside cancer treatment
  • Anti-inflammatory
  • General wellness/tonic
  • Antimicrobial support

Lentinan is a beta-glucan polysaccharide from shiitake mushrooms that is studied mainly as an immune-modulating add-on to cancer chemotherapy, where it is given by injection in a few countries. Evidence for oral supplements is limited, and quality varies widely between products. Talk with your pharmacist or doctor before using it, especially if you have a health condition or are pregnant or breastfeeding.

About Warfarin

What Is Warfarin Used For?

Brand names: Coumadin, Panwarfin, Sofarin

Warfarin helps prevent and treat harmful blood clots. The label lists these uses for both Warfarin Sodium tablets and Jantoven tablets:

  • Preventing and treating venous thrombosis (clots in the veins) and pulmonary embolism (clots in the lungs).
  • Preventing and treating clot-related complications of atrial fibrillation and/or heart valve replacement.
  • Lowering the risk of death, another heart attack, and clot-related events such as stroke after a heart attack.

Warfarin does not dissolve a clot that has already formed. It helps stop the clot from growing and prevents further clot-related complications.
