Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the deal with this combo. The dextromethorphan in Vicks Formula 44D is cleared by a liver enzyme called CYP2D6. Lab research hints that lentinan might speed that enzyme up. If that happened in real life, your body could break dextromethorphan down a little faster, which might make the cough-suppressing effect feel weaker or shorter. The phenylephrine piece is mostly handled by a different pathway, so it isn't really the player here.

I want to keep this in perspective: this is only test-tube and animal data, and nothing like it has been reported in actual people. So I wouldn't lose sleep over an occasional cough medicine. If you take lentinan regularly, just mention it to your pharmacist or doctor first.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with Vicks Formula 44D. One of its ingredients, dextromethorphan (the cough suppressant), is partly broken down by a liver enzyme called CYP3A4. Lab and animal studies hint that lentinan may speed up that enzyme. If that happened in people, your body might clear the dextromethorphan a bit faster, which could make the cough relief feel weaker than usual.

I want to be honest about the evidence here: this is only test-tube and animal data, and it has never been reported in real patients. So I wouldn't lose sleep over it. The phenylephrine (decongestant) part isn't really driven by this enzyme. If you use both regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.
