Codeine, Papaverine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about codeine (the opioid half of Copavin): it's actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain and calms a cough. So the enzyme isn't clearing codeine away, it's switching it on.
Lab research hints that lentinan might speed up CYP2D6. If that held true in people, it could mean more codeine gets turned into morphine, which could nudge up opioid effects like drowsiness, constipation, or slowed breathing. But honestly, this is only test-tube and animal data, and it has never been shown in humans, so I wouldn't lose sleep over it. Just mention it to your pharmacist or doctor before combining them.
