Why this applies to Quetiapine

Quetiapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the situation with shiitake-derived lentinan and your quetiapine (Seroquel). Lab studies hint that lentinan might speed up an enzyme called CYP2D6, which is one of the pathways your body uses to clear quetiapine. If that enzyme works faster, your quetiapine could be cleared a little quicker, which in theory means slightly lower levels and possibly a weaker effect on your mood or thinking symptoms.

The important part: this has only been seen in test-tube and animal research, never reported in real patients, so it's very theoretical. Quetiapine is actually broken down mainly by a different enzyme (CYP3A4), so the real-world impact is likely small. Still, if you notice your symptoms creeping back, mention it to your pharmacist or doctor before changing anything.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Your body relies heavily on an enzyme called CYP3A4 to break down quetiapine. In test-tube studies, lentinan seems to speed that enzyme up. If that holds true in people, your body could clear quetiapine a bit faster, which might leave less of the medication working in your system.

What could that look like in real life? Possibly a return or worsening of the symptoms quetiapine is keeping in check, such as mood swings, trouble sleeping, or unsettled thinking. I want to be honest with you though: this has only been seen in lab and animal research, not in actual patients, so it's more of a theoretical heads-up than a proven problem. Please don't change your dose on your own. Just loop in your pharmacist or doctor before adding lentinan so they can keep an eye on how you're doing.

What Is Quetiapine Used For?

Brand names: Seroquel, Dopaquel

Quetiapine treats schizophrenia and bipolar disorder in adults and certain age groups of children and adolescents. For bipolar disorder, it is approved for manic episodes (both as a stand-alone treatment and alongside lithium or divalproex) and for depressive episodes.

The extended-release form of quetiapine is also approved as add-on therapy for major depressive disorder (MDD) in adults who are already taking an antidepressant. The regular tablet form is not approved for this use.
