Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the gist for propranolol: your body relies on a liver enzyme called CYP1A2 to break it down. Lab and animal studies hint that lentinan may speed that enzyme up. If that held true in people, your body could clear propranolol a little faster, which might leave less of it working than expected. Propranolol is an active drug (not a prodrug), so faster clearance points toward a weaker effect, not a stronger one.

I want to be honest about how thin this evidence is. This has only been seen in test tube and animal work, never reported in actual patients, so I wouldn't lose sleep over it. Still, since the oral solution (Hemangeol) is dosed carefully for tiny infants, please run any lentinan use past your pediatrician or pharmacist first.

Here's the gist for propranolol (Hemangeol). Lab studies hint that lentinan may speed up an enzyme called CYP2C19, one of the liver pathways that helps break down propranolol. If that happened in real life, your child's body could clear the medicine a bit faster, which might make it work less well. With an infant hemangioma, weaker beta-blocker effect could mean the growth responds less than expected.

The reassuring part: this is only based on test-tube and animal research, and it has never actually been reported in people. Propranolol is not a prodrug, so the direction here is reduced effect, not buildup. Still, before giving any lentinan alongside your baby's propranolol, please check with your pharmacist or doctor first.

Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for propranolol (Hemangeol), which is the form used to treat infantile hemangiomas. Your baby's body relies on a liver enzyme called CYP2D6 to break propranolol down. In test tube studies, lentinan seemed to speed up that enzyme. If that happened in a real person, the body might clear propranolol a little faster, which could leave less of the drug working and the hemangioma treatment less effective.

I'll be honest: this is only lab-bench evidence and has never actually been seen in people, so I wouldn't lose sleep over it. Still, lentinan isn't something I'd give a young infant on heart medication without checking first. Please run it by your pediatrician or pharmacist before mixing the two.

What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
