Metoprolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for metoprolol: your liver uses an enzyme called CYP2D6 to break this drug down. Lab and animal studies hint that lentinan might rev up that enzyme a bit. If that happened in people, your body could clear metoprolol a little faster, meaning slightly weaker effects. In real life, that might look like your heart rate or blood pressure creeping back up, or chest pain feeling less controlled.

I want to be honest, though: this has only been seen in test-tube and animal research, never in actual patients, so it's mostly theoretical. I wouldn't lose sleep over it. Still, since metoprolol manages important heart issues, check with your pharmacist or doctor before adding lentinan, and don't change your dose on your own.

Here's the gist for you. Your body clears metoprolol mainly through a liver enzyme called CYP2D6. In animal studies, lentinan seems to speed that enzyme up, which could push metoprolol out of your system faster and lower the amount in your blood. Metoprolol is not a prodrug, so faster clearance simply means less drug doing its job.

In real terms, that might make metoprolol less effective, so your blood pressure or heart rate could creep back up, or chest pain could become harder to control. I want to be honest though: this is based only on animal data, so we don't really know if it matters in people. Please don't stop or change anything on your own. Just loop in your pharmacist or doctor before adding lentinan.

About Metoprolol

What Is Metoprolol Used For?

Brand names: Lopressor, Toprol XL

Metoprolol is used to treat several heart and blood pressure conditions. Metoprolol succinate extended-release tablets (including Toprol-XL) are approved for high blood pressure, angina pectoris (chest pain from the heart), and heart failure. Metoprolol tartrate tablets (including Lopressor) are approved for high blood pressure, angina, and — when used alongside intravenous metoprolol — to reduce the risk of cardiovascular death after a definite or suspected heart attack.

Metoprolol tartrate injection (intravenous form) is used in hemodynamically stable patients with a definite or suspected acute heart attack to reduce cardiovascular death, and is followed by oral metoprolol therapy. Lowering blood pressure with metoprolol reduces the risk of strokes and heart attacks.
