Why this applies to Selinexor

Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the short version for your situation. Selinexor is processed in part by a liver enzyme called CYP3A4. Lentinan, in lab studies only, seems to speed up that enzyme. If that were to happen in people, your body could clear selinexor a little faster, which might lower its blood levels and, in theory, make it work less well against your myeloma or lymphoma.

I want to be honest about how soft this evidence is. This comes from test-tube and animal research, and it has never been reported in actual patients. Still, selinexor is a serious cancer treatment, so I wouldn't add lentinan on your own. Please run it by your oncologist or pharmacist first so nothing weakens your therapy.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
