Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist with Selinexor (Xpovio) and adrafinil. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4, which is one of the pathways your body uses to break down selinexor. If selinexor is cleared a little faster, its blood levels could dip and it may not fight your myeloma or lymphoma as well as it should.

I want to be honest about the strength of this: it's theoretical, based on how these drugs behave rather than real cases in people, and modafinil is only a weak inducer. Still, selinexor is serious cancer therapy, so I wouldn't pair it with an unregulated supplement on your own. Please run this by your oncology team or pharmacist first.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
