Hydrocodone Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with hydrocodone: your body relies on an enzyme called CYP2D6 to convert it into hydromorphone, the form that does most of the heavy lifting for pain relief. In other words, hydrocodone is partly a prodrug that needs activating.

Lab research hints that lentinan might speed up CYP2D6. If that held true in people, your body could convert hydrocodone faster and possibly clear it quicker, which in theory might mean weaker or shorter-lasting pain control. But honestly, this has only been seen in test-tube and animal studies, never reported in real patients, so I wouldn't lose sleep over it. Still, since you're on round-the-clock pain therapy, mention the lentinan to your pharmacist or doctor and watch for any unexpected change in how well your pain is managed.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for hydrocodone: your liver uses an enzyme called CYP3A4 to help break it down. Lab and animal studies hint that lentinan may speed up that enzyme. If that holds true in people, your body could clear hydrocodone a bit faster than usual, which might mean weaker pain relief from the same dose.

I want to be honest about the strength of this, though. It's only test-tube and animal data, and it hasn't been seen in actual patients, so this is more of a theoretical concern than a proven problem. Since hydrocodone is used for serious, around-the-clock pain, any drop in relief matters. Please don't change your dose on your own. Just give your pharmacist or doctor a heads-up if you're considering lentinan, and watch for pain that creeps back.
