Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the basic idea with imipramine (Tofranil). Your liver uses an enzyme called CYP1A2 as one way to clear this antidepressant out of your body. Lentinan, the shiitake mushroom extract, may speed that enzyme up. If it does, your body could break down imipramine a little faster, which can lower the amount in your blood and possibly make it work less well. For depression that might mean your mood symptoms creep back, and for a child using it for bedwetting it could become less effective.

The important caveat: this is based only on lab data, not real people, so it's more of a theoretical heads-up than a proven problem. Imipramine still needs careful dosing, so before adding lentinan, please check with your pharmacist or doctor.

Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the gist for your imipramine (Tofranil). One of the liver enzymes that helps process this antidepressant is called CYP2C19. Lab and animal research hints that lentinan might speed up that enzyme, which in theory could clear imipramine from your body a little faster and leave less of it working. If that happened, you might notice your depression symptoms creeping back, or in a child, bedwetting returning.

I want to be honest about the strength here: this is only test-tube and animal data, and it has never actually been reported in people. So I wouldn't lose sleep over it. Still, since imipramine needs steady levels to work well, just give your pharmacist or doctor a heads-up before adding lentinan so they can keep an eye on how you're doing.

Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
