Why this applies to Salmeterol

Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for your Serevent inhaler. Salmeterol gets cleared from your body by a liver enzyme called CYP3A4. Lab studies hint that lentinan might speed that enzyme up a bit. If that happened in real life, your body could break salmeterol down a little faster, which in theory could make it work less well, so you might notice more wheezing, coughing, shortness of breath, or chest tightness.

I want to be honest about the evidence here: this comes only from test-tube and animal research, and it has never been reported in actual people. So this is more of a heads-up than a red alert. Salmeterol isn't a prodrug, so it doesn't flip the usual direction. Still, please run lentinan past your pharmacist or doctor before starting it, especially since it can affect your immune system.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
