Salmeterol (Serevent) is the long-acting inhaler taken twice daily for asthma or COPD, and its supplement list is one I would approach with real caution. Seven of the 141 ingredients on file rate major: grapefruit can raise salmeterol levels by blocking the enzyme that clears it, St. John's wort revs that same enzyme and can weaken the drug, and several botanicals can stretch the heart's QT rhythm on top of salmeterol's own tendency to do so, raising the odds of a dangerous arrhythmia.
