American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with pairing American Hellebore and Salmeterol (Serevent). Salmeterol is a long-acting inhaler that opens your airways, but like other beta agonists, it can nudge your heart's electrical rhythm and slightly stretch what's called the QT interval. American Hellebore's toxic alkaloids can do the same thing on their own. Stack two QT-stretching effects together and you raise the risk of a rare but dangerous heart rhythm called torsades de pointes, which can cause fainting, a racing or fluttering heartbeat, or worse.

This isn't a prodrug situation, it's an additive heart-rhythm risk. The evidence here is fairly limited, but the potential outcome is serious. On top of that, American Hellebore is genuinely poisonous and isn't a safe supplement to use. Please don't combine these without talking to your pharmacist or doctor first.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
