Salmeterol falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with pairing ephedra and Salmeterol (Serevent). Both can stress your heart in similar ways. Salmeterol is a long-acting beta agonist, and like other drugs in its class it can slightly nudge your heart's electrical timing (what's called the QT interval) and sometimes speed up your heartbeat. Ephedra is a strong stimulant that does much the same thing, raising heart rate and blood pressure.

Stack them together and those effects can add up, which may raise the risk of an irregular or racing heartbeat, palpitations, jitteriness, or feeling lightheaded. The evidence here isn't rock solid, but it's plausible enough to take seriously, especially since ephedra is banned in U.S. supplements for safety reasons. Please don't change your Salmeterol, but do talk with your pharmacist or doctor before using anything with ephedra in it.

Here's the gist for your inhaler. Salmeterol (Serevent) is a long-acting beta agonist, meaning it gently stimulates the same kind of receptors that ephedra's stimulants (ephedrine and pseudoephedrine) hit. When you stack two things that both rev up that system, the effects can add together. In plain terms, your heart could pound harder or faster, you might feel jittery or shaky, and your blood pressure could climb.

This concern is theoretical and based on how the drugs work, not on strong studies, so I don't want to alarm you. Still, ephedra is banned in U.S. supplements for good reason, and combining it with salmeterol isn't worth the risk. Please skip ephedra and talk with your pharmacist or doctor about safer choices.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
