Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the short version: your salmeterol (Serevent) is partly cleared by a liver enzyme called CYP3A4, and Boswellia may interfere with that enzyme. The catch is that this is purely a theoretical concern from lab-dish studies, not something proven in real people. Boswellia mostly seems to slow this enzyme, which could let a little more salmeterol linger in your system.

If that happened, you might notice more of salmeterol's usual side effects, like a faster or pounding heartbeat, jitteriness, shakiness, or a headache. The evidence here is weak, so I wouldn't lose sleep over it. Still, since salmeterol affects the heart, it's worth a quick chat with your pharmacist or doctor before adding Boswellia, especially the concentrated supplement form.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
