Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist with boswellia and prednisolone: your body uses a liver enzyme called CYP3A4 to help clear prednisolone out of your system. In lab studies (test tube, not people), boswellic acids mostly seem to slow that enzyme down, which could in theory let prednisolone build up a bit. If that happened, you might notice more of the usual steroid effects, things like trouble sleeping, mood swings, a bigger appetite, fluid retention, or higher blood sugar.

I want to be straight with you though: this is theoretical, based only on pharmacology, not real patient reports. For most people it's unlikely to cause a noticeable problem. Still, since the lab data is mixed, it's worth a quick chat with your pharmacist or doctor before you combine them, especially on higher steroid doses.

Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.

Here's the honest version: when it comes to mixing Boswellia with prednisolone, we're working with a pretty thin amount of evidence. The concern is purely theoretical and comes from lab dish and animal studies, not from people taking the two together. So I don't want you to lose sleep over this one.

Prednisolone is a corticosteroid that calms your immune system and inflammation. Boswellia also nudges inflammation pathways, and in some lab work it appeared to dampen immune activity while in other studies it seemed to stimulate it. Because the signals point in both directions, nobody can say for sure whether it would strengthen or weaken your steroid. If you're on prednisolone for something serious like preventing transplant rejection or a tough autoimmune flare, please run it by your pharmacist or doctor before adding Boswellia.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
