Here's the simple version. Salmeterol (Serevent) is a long-acting inhaler that relaxes and opens your airways. One of its known effects is mild stimulation of the heart, which can sometimes show up as a faster or more forceful heartbeat, a fluttery feeling, or jitteriness. Yerba mate brings its own kick of caffeine, and caffeine nudges the heart in the same direction.

So when you stack the two, the worry is simply additive: a little more pep on the heart than either one alone. This is a theoretical concern based on how the two work, not something proven in studies, and it's more likely with large amounts of mate rather than a normal cup or two. If you notice a racing heart, shakiness, or feel wired, ease up on the mate and chat with your pharmacist or doctor.

Salmeterol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the gist for your Serevent inhaler. Salmeterol is cleared from your body by a liver enzyme called CYP3A4. In lab (test tube) studies, yerba mate appears to slow that enzyme down a little. In theory, that could let salmeterol build up slightly, which might nudge up its usual side effects, things like a faster or pounding heartbeat, jitteriness or tremor, and sometimes a headache.

I want to keep this in perspective, though. This is based only on early lab work, not on real people, and it's rated as a minor interaction. On top of that, yerba mate brings its own caffeine, which can add to that wired, racing-heart feeling. If you notice your heart fluttering or you feel shaky after combining them, mention it to your pharmacist or doctor. Don't change how you use your inhaler on your own.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
