Theoretically, the caffeine in yerba mate may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Yerba mate contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, it might increase the risk of bleeding when used concomitantly with these agents; however, this interaction has not been reported in humans.

Here's the short version: dipyridamole already works to keep your platelets from sticking together so clots don't form too easily. The caffeine in yerba mate has shown some mild platelet-slowing activity too, at least in lab studies. So in theory, pairing the two could nudge your bleeding risk up a little, meaning things like easier bruising, nosebleeds, or bleeding gums.

I want to be honest with you though: this is based only on test-tube and animal research, and it has never actually been reported in people. A cup or two of mate is very unlikely to cause trouble. If you drink it heavily every day, it's worth mentioning to your pharmacist or doctor, especially since you're on dipyridamole after a valve replacement. Don't change anything on your own.

Theoretically, the caffeine in yerba mate might decrease the vasodilatory effects of dipyridamole and interfere with its use prior to stress testing.
Yerba mate contains caffeine. Caffeine inhibits dipyridamole-induced vasodilation. Still, some researchers recommend that methylxanthines, such as caffeine, as well as methylxanthine-containing products, should be stopped 24 hours prior to pharmacological stress. Methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.

Here's the key thing about dipyridamole: part of how it works is by widening (relaxing) your blood vessels. The caffeine in yerba mate works against that, because caffeine blocks the same pathway dipyridamole uses to open vessels up. So the worry isn't that the drug builds up or becomes dangerous, it's that yerba mate can blunt dipyridamole's effect.

This matters most when dipyridamole is given as part of a pharmacologic cardiac stress test. If caffeine is in your system, the test may not work properly and could give a misleading result. That's why many clinics ask you to avoid caffeine, including yerba mate, for about 24 hours beforehand. For everyday use, talk with your pharmacist or doctor before changing anything.

What Is Dipyridamole Used For?

Brand names: Persantine, Dipyridamole, Persantin, Persantin Injection, Persantin Retard

Dipyridamole tablets are used alongside warfarin (a blood thinner) to help prevent dangerous blood clots from forming after cardiac valve replacement surgery. It is not used alone for this purpose — it works as an add-on to anticoagulant therapy.

Dipyridamole Injection is used as an alternative to exercise during thallium myocardial perfusion imaging — a type of heart scan that checks blood flow through the coronary arteries. It is given to patients who are unable to exercise adequately for the test.
