Theoretically, concomitant use of CYP1A2 inhibitors and yerba mate might increase levels and adverse effects of the caffeine in yerba mate.
Yerba mate contains caffeine. Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the thing to know about this pairing: it's really the caffeine in your yerba mate that gets affected, not the antibiotic itself. Erythromycin can mildly slow down a liver enzyme called CYP1A2, which is the same enzyme your body uses to clear caffeine. When that enzyme is a little slower, caffeine can hang around longer and build up.

In plain terms, while you're on erythromycin, your usual mug of mate might hit harder than expected. Think jitteriness, a racing heart, trouble sleeping, or feeling on edge. This is theoretical and rated minor, so most people won't notice much. Erythromycin itself can already upset the stomach, so go easy on the mate, and check with your pharmacist if you feel wired.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the basic idea: your body uses an enzyme called CYP3A4 to help clear erythromycin out of your system. Lab studies (in test tubes, not people) suggest yerba mate may slow that enzyme down a bit. In theory, that could let erythromycin linger a little longer and nudge up its usual side effects, like upset stomach, nausea, cramping, or diarrhea.

I want to be honest with you, though: this is theoretical and based only on lab data, so it's a minor concern at most. A cup or two of mate is unlikely to cause trouble. If you tend to drink a lot of it, or you take concentrated mate supplements while on erythromycin, just give your pharmacist or doctor a quick heads-up.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
