Use of ephedrine with caffeine can increase the risk of stimulatory adverse effects. There is evidence that using ephedrine with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction, stroke, seizures, and death.

Here's the thing to watch with this combo. Your medication already contains two stimulants: ephedrine and aminophylline (a theophylline-type drug). Yerba mate adds its own caffeine on top, and caffeine and ephedrine are well known to amplify each other. Stacking them can push your heart rate and blood pressure higher and leave you feeling jittery, anxious, or shaky, with a racing or pounding heartbeat and trouble sleeping. In more serious cases this kind of stimulant overload has been tied to dangerous spikes in blood pressure, irregular heartbeats, and seizures.

The evidence here is mostly anecdotal, but because the effect direction is consistent and potentially serious, I'd be cautious. A casual cup of mate is different from concentrated mate supplements. Please check with your pharmacist or doctor before pairing them.

Here's the thing to keep in mind: this old combo product already contains two stimulants. Aminophylline is a theophylline-based bronchodilator, and ephedrine revs up the nervous system too (the amobarbital is added to take the edge off). Yerba mate brings its own caffeine to the party, and caffeine and theophylline are chemical cousins. Stacking them can pile on stimulant effects.

In real life that might mean a racing or pounding heart, jitteriness, trouble sleeping, tremor, headache, or an upset stomach. This is based mostly on expert opinion rather than hard studies, so it's a caution, not a crisis. A cup now and then is likely milder than concentrated mate supplements. Before mixing them regularly, check with your pharmacist or doctor.
