Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tree Turmeric interacts with.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.

Your salmeterol inhaler (Serevent) is cleared from the body mainly by a liver enzyme called CYP3A4. The berberine in tree turmeric can slow that enzyme down. If salmeterol gets broken down more slowly, a bit more of it can hang around, and that can nudge up the typical side effects of this kind of medicine: a faster or pounding heartbeat, jitteriness or shakiness, headache, or muscle cramps. Salmeterol is not a prodrug, so more in the body means a stronger effect, not a weaker one.

To be fair, this is mostly based on lab work and early human data, so the real-world impact is uncertain. Still, if you take an inhaled steroid plus salmeterol regularly, it is worth a quick chat with your pharmacist or doctor before adding a berberine supplement.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
