ANTIHYPERTENSIVE DRUGS

Diltiazem falls under ANTIHYPERTENSIVE DRUGS — the group Tree Turmeric interacts with.
Animal research suggests that berberine, a constituent of tree turmeric, can have hypotensive effects. Also, a meta-analysis of clinical research suggests that taking berberine in combination with amlodipine (Norvasc) can lower systolic and diastolic blood pressure when compared with taking amlodipine alone.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: tree turmeric (the berberine in it) may gently lower blood pressure on its own. Diltiazem already relaxes your blood vessels and slows the heart to treat high blood pressure and angina. Stack the two and their effects could add up, so your blood pressure might dip lower than you want.
If that happens, you could feel dizzy, lightheaded, tired, or notice your heart rate running slow, especially when standing up quickly. I'll be honest though, this is mostly theoretical, leaning on animal studies and a berberine trial with a different drug (amlodipine), so it's not a sure thing. Don't change anything on your own. Just give your pharmacist or doctor a heads-up and keep an eye on your blood pressure if you decide to use both.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Diltiazem also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down diltiazem and keep its levels in a comfortable range. The berberine in tree turmeric can partly block that enzyme. With the cleanup slowed, more diltiazem may linger in your bloodstream, which can nudge its effects up a notch.
For diltiazem, that might show up as lower blood pressure than expected, dizziness, a slower heart rate, swollen ankles, or feeling unusually tired. This concern is reasonable but still partly theoretical, so don't panic. Just loop in your pharmacist or doctor before combining the two, and let them know if you notice any of those symptoms so they can keep an eye on your readings.

Oral extended-release diltiazem (capsules and tablets) treats high blood pressure (hypertension). It is also used for chronic stable angina, which is chest pain from the heart working harder than its blood supply allows. It can be used alone or with other blood pressure medicines.
Some extended-release capsule products, such as Cartia, Cartia Xt and certain generic diltiazem capsules, are also approved for angina due to coronary artery spasm. The intravenous (IV) injection is a different use: it gives temporary control of a rapid heart rate in atrial fibrillation or atrial flutter, and rapidly converts paroxysmal supraventricular tachycardia (PSVT) to normal rhythm.
