Tree Turmeric + Dextromethorphan
Dextromethorphan brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough.
Dextromethorphan falls into 3 drug categories that Tree Turmeric can interact with; the most serious is rated moderate. Theoretically, tree turmeric might increase the levels and clinical effects of drugs metabolized by CYP2D6.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Tree Turmeric interacts with whole categories of medication. Dextromethorphan falls into 3 of those categories, so it can interact with Tree Turmeric in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 3Why this applies to Dextromethorphan
Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Tree Turmeric interacts with.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears dextromethorphan mostly through a liver enzyme called CYP2D6. The berberine in tree turmeric appears to slow that enzyme down. When CYP2D6 is blocked, dextromethorphan isn't broken down as quickly, so more of it can hang around in your system. Dextromethorphan is the active drug itself (not a prodrug), so higher levels usually mean stronger effects.
In real life that could mean more drowsiness, dizziness, nausea, or feeling foggy or restless. The good news is this is based on early, lower-quality evidence, so it's a possible concern rather than a sure thing. If you take a concentrated berberine supplement regularly, mention it to your pharmacist or doctor before reaching for cough syrup.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Dextromethorphan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist with tree turmeric (berberine) and your cough medicine. Berberine can gently slow down a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear dextromethorphan. When that pathway gets blocked, more of the drug can hang around, so you may feel its effects a bit more strongly.
For dextromethorphan, that can mean extra drowsiness, dizziness, lightheadedness, or feeling foggy or restless. The good news is this is usually a moderate, manageable concern, and short-term cough use is unlikely to cause big trouble for most people. Still, since concentrated berberine supplements are much stronger than a kitchen spice, it's worth running this combo by your pharmacist or doctor first.

DEXTROMETHORPHAN (Robitussin DM, others)
3 of 3Preliminary clinical research suggests that berberine, a constituent of tree turmeric, can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your body relies on a liver enzyme called CYP2D6 to break down dextromethorphan, the cough suppressant in products like Robitussin DM and Benylin. The berberine in tree turmeric appears to slow that enzyme down. When the enzyme works slower, the cough medicine clears out of your system less efficiently and can build up a bit higher than usual.
In real terms, that could mean more of dextromethorphan's typical effects, like feeling drowsy, foggy, dizzy, or unusually restless. The good news is the evidence here is fairly limited and the effect is rated moderate, not severe. Still, if you take berberine or tree turmeric regularly, it's worth a quick chat with your pharmacist or doctor before adding a cough product, rather than changing anything on your own.
