Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the gist with resveratrol and your cough medicine. Dextromethorphan is partly cleared by a liver enzyme called CYP3A4, and resveratrol has shown in lab dishes that it can slow that enzyme down. In theory, if that enzyme works slower, a bit more dextromethorphan could hang around, which might mean more of its usual effects like drowsiness, dizziness, or feeling lightheaded.

Honestly though, this is mostly test-tube science. In actual people, even high-dose resveratrol (3000 mg a day for 8 weeks) did not force any changes to CYP3A4 medications. So for short-term cough use, I wouldn't lose sleep over it. Still, if you take resveratrol regularly, it's worth a quick word with your pharmacist or doctor.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
