Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the thinking on this pair. Your body clears cyclosporine using an enzyme called CYP3A4. In lab dish studies, resveratrol can slow that enzyme down. If that happened in real life, cyclosporine could build up in your blood and push you toward more side effects, like higher blood pressure, kidney strain, tremor, headache, or swollen gums. Cyclosporine is a drug where blood levels really matter, so even small shifts get our attention.

The reassuring part: this concern comes mainly from test-tube research. In an actual human study, resveratrol at 3000 mg daily for 8 weeks didn't force changes to CYP3A4 medications. Still, please loop in your transplant team or pharmacist before adding resveratrol, since your levels are usually monitored.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
