Prednisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to clear prednisone. In test-tube studies, resveratrol can slow that enzyme down a bit. On paper, that could let prednisone hang around a little longer and nudge up its effects, things like fluid retention, higher blood sugar, mood or sleep changes, or stomach upset.

The reassuring part is that this is mostly theoretical. The lab signal hasn't really panned out in people, and one human study using a hefty 3000 mg of resveratrol daily for 8 weeks found no need to adjust CYP3A4 medications. So for most folks this isn't a big worry. Still, since prednisone doses are personalized, just give your pharmacist or doctor a quick heads-up before adding resveratrol.
