Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with grape seed extract and your cough medicine. Your body relies on a liver enzyme called CYP2D6 to break down dextromethorphan and clear it out. Lab tests (done in test tubes, not in people) hint that concentrated grape seed extract might slow that enzyme down a bit. If that happened, dextromethorphan could linger longer and feel stronger, possibly bringing more drowsiness, dizziness, or an upset stomach.
Now the reassuring part: this has never actually been seen in real people, so it's a theoretical concern at best. Eating grapes won't do this. If you're using a high-dose grape seed supplement and a cough product together, just give your pharmacist a quick heads-up so they can take a look.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture with grape seed extract and your cough medicine. Dextromethorphan is partly handled by a liver enzyme called CYP3A4, and grape seed extract may nudge that enzyme. The catch is that the research can't agree on which direction it goes. Some lab studies hint it slows the enzyme down, while animal studies suggest it might speed it up.
If it slowed clearance, dextromethorphan could build up a bit, possibly bringing on dizziness, drowsiness, or an upset stomach. If it sped things up, your cough relief might feel weaker. Either way, this is only theoretical and hasn't actually been seen in people. Eating grapes won't matter. If you're considering a grape seed supplement, just run it by your pharmacist first.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation. This can happen with the common cold or from inhaled irritants.
Some products are also labeled to help you get to sleep by reducing the urge to cough. Some labels also list helping reduce how intense the coughing is. Honey Works syrup, Premier Value Tussin Cough and CVS Long Acting Cough liquids are also labeled to help loosen phlegm (mucus) and thin bronchial secretions so they drain from the bronchial tubes.
