Grape and Colchicine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Colchicine

Colchicine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with grape seed extract and colchicine. Your body relies on an enzyme called CYP3A4 to help clear colchicine out of your system. Grape seed extract may tinker with that enzyme, but the lab and animal studies disagree on which direction it goes. Some test-tube work suggests it slows the enzyme down (which could let colchicine build up), while animal data hints it might speed the enzyme up (which could lower colchicine levels).

Colchicine is a drug where the dose really matters, since too much can cause nausea, diarrhea, belly cramps, and muscle problems. The good news: this interaction has not actually been seen in people, so it stays theoretical for now. Still, if you want to add a grape supplement, run it by your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

“Colchicine, the gout and pericarditis medicine sold as Colcrys and Mitigare, has one of the smallest gaps between a helpful dose and a harmful one of any drug I dispense, so its 146 supplement interactions deserve genuine respect. Most of them work the same way, blocking or speeding up the enzyme and transporter (CYP3A4 and P-glycoprot…”
