Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Grape interacts with.

It is unclear if grape juice or grape seed extract inhibits CYP2C9; research is conflicting.
In vitro evidence shows that grape seed extract or grape juice might inhibit CYP2C9 enzymes. However, a small pharmacokinetic study in healthy adults shows that drinking 8 ounces of grape juice once does not affect the clearance of flurbiprofen, a probe-drug for CYP2C9 metabolism. The effects of continued grape juice consumption are unclear.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to break down phenytoin (Dilantin). There's some lab-dish evidence that compounds in grape juice or grape seed extract might slow that enzyme down. If that happened, phenytoin could clear a little slower and build up, which can mean things like drowsiness, dizziness, unsteady walking, slurred speech, or jerky eye movements.

That said, the real-world evidence here is thin. A small human study found that an 8-ounce glass of grape juice didn't change how the body handled a similar CYP2C9 drug. Phenytoin is one of those medicines where levels need to stay in a narrow window, so even small shifts matter. Enjoying grapes as food is fine, but check with your pharmacist or doctor before using concentrated grape supplements.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
