Phenytoin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with goji and phenytoin (Dilantin). Your liver uses an enzyme called CYP2C19 to help break phenytoin down. Lab studies suggest goji may slow that enzyme a bit. In theory, that could let phenytoin build up in your blood, and since phenytoin has a narrow safety window, even small increases can tip you into side effects like dizziness, drowsiness, slurred speech, unsteady walking, or jerky eye movements.
The important part: this is only a test-tube finding, and it has never actually been reported in people. Food amounts of goji berries are very unlikely to cause trouble. With concentrated goji supplements I'd be more cautious. Please loop in your pharmacist or doctor before adding one, since phenytoin levels are easy to check.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for you. Your body relies on a liver enzyme called CYP2C9 to break down phenytoin and keep its level steady. Concentrated goji products (think strong teas, juices, or supplement extracts) may slow that enzyme down a bit. If phenytoin clears more slowly, its level can creep up, and phenytoin is a finicky drug where even a small bump can tip you into side effects like dizziness, unsteady walking, slurred speech, double vision, drowsiness, or nausea.
That said, this is a theoretical concern. It's based mostly on lab studies and reports with warfarin (a similar enzyme partner), not phenytoin directly. Eating a normal handful of goji berries is unlikely to matter. Before adding a concentrated goji supplement, run it by your pharmacist or doctor, since your phenytoin level may need a check.

What Is Phenytoin Used For?
Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection
Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.
Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
