Goji and Ibuprofen

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Ibuprofen

Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body clears ibuprofen partly through a liver enzyme called CYP2C9. Goji may slow that enzyme down, so in theory ibuprofen could hang around a little longer and build up. If that happens, you might notice more of the usual ibuprofen side effects, things like stomach upset, heartburn, or in heavier or longer use, a higher chance of stomach irritation or kidney strain.

That said, I want to keep this in perspective. The evidence here is weak, mostly lab studies plus a few case reports involving warfarin, not ibuprofen itself. Enjoying goji berries as a food is very unlikely to cause trouble. The bigger question is concentrated goji juice, tea, or supplements taken regularly. If that's you, just check in with your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).

Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
