Goji and Temazepam

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Temazepam

Temazepam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your body uses a liver enzyme called CYP3A4 to help clear temazepam. Lab studies hint that goji berry might slow that enzyme down. If that happened in a real person, temazepam could hang around a bit longer than usual, which could mean more drowsiness, grogginess, dizziness, or feeling unsteady the next morning.

The good news is this is only a theoretical concern. It comes from test-tube research, and it has never actually been reported in people. Eating goji berries as a food is very unlikely to cause a problem. If you're using concentrated goji supplements or goji juice regularly, just mention it to your pharmacist or doctor so they can keep an eye on how rested (or groggy) you feel.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Temazepam page

“Alcohol is the interaction I would flag first for anyone taking temazepam (Restoril), a benzodiazepine sleep medicine. Beer and wine both carry major ratings because they layer their own sedation on top of a drug that already depresses the nervous system, and that additive drowsiness runs through 78 of the 197 supplements on file.”
