Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the gist with goji and cyclosporine. Your body clears cyclosporine using a liver enzyme called CYP3A4. Lab studies suggest goji may slow that enzyme down a bit. If that happens, cyclosporine could clear more slowly and build up in your blood, which can mean higher levels and a greater chance of side effects like kidney strain, high blood pressure, tremor, or headache.

Now the reassuring part: this is only seen in test-tube research, not in actual people, so it's more of a theoretical caution than a proven problem. That said, cyclosporine is a finicky drug where even small swings in level matter, especially for transplant patients. I wouldn't add a concentrated goji supplement without first checking with your transplant team or pharmacist. An occasional handful of berries is far less concerning.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
