These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Goji interacts with whole categories of medication. Dextromethorphan falls into 2 of those categories, so it can interact with Goji in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Dextromethorphan
Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with goji and your cough medicine. Your body relies on a liver enzyme called CYP2D6 to clear dextromethorphan out of your system. Lab studies (done in test tubes, not in people) suggest goji berry may slow that enzyme down. If that happened in real life, dextromethorphan could hang around longer and build up a bit, which might mean more drowsiness, dizziness, or feeling foggy or restless.
The honest part: this is purely theoretical right now and has never actually been reported in humans, so I wouldn't lose sleep over it. Eating goji berries as food is very unlikely to matter. If you're taking concentrated goji supplements along with cough medicine, just mention it to your pharmacist to be safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with goji and your cough medicine. In lab dishes, goji juice slows down an enzyme called CYP3A4, which is one of the pathways your body uses to break down dextromethorphan. If that pathway is dialed back, a little more of the drug could hang around, which might mean more drowsiness, dizziness, or a bit of stomach upset than usual.
The honest part: this has only been seen in a test tube, never reported in real people, so I'd put it low on the worry list. Eating goji berries as food is very unlikely to cause trouble. If you're using concentrated goji supplements regularly, just give your pharmacist a heads-up so we can keep an eye on how you feel.
