PHENYTOIN (Dilantin)

Theoretically, black tea might reduce the effects of phenytoin and increase the risk for convulsions.
Black tea contains caffeine. Animal research suggests that caffeine can decrease the anticonvulsant activity of phenytoin. The effect does not seem to be related to the seizure threshold-lowering effects of caffeine. However, the exact mechanism of this interaction is unclear.

Here's the situation with your daily cup of black tea and phenytoin (Dilantin). Phenytoin is a seizure medicine that works directly in the brain, and it has what we call a narrow therapeutic window, meaning the right blood level sits in a fairly tight range. The concern on file is that the caffeine in black tea might slightly blunt how well phenytoin holds seizures back, which in theory could make a breakthrough seizure more likely.

I want to be honest about how soft this evidence is. It comes from animal and lab studies, not from people, and the exact reason isn't well understood. A moderate tea habit is unlikely to cause trouble for most folks. Still, since phenytoin is a drug where small shifts matter, mention your tea or caffeine intake to your pharmacist or doctor before changing anything.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
