Why this applies to Levofloxacin

Levofloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Black Tea interacts with.

Theoretically, black tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.
In vitro, black tea extract inhibits organic anion-transporting polypeptide (OATP)2B1. OATP2B1 is expressed in the small intestine and liver and is responsible for the uptake of drugs and other compounds. In an animal model, black tea extract was found to inhibit the absorption of rosuvastatin, a substrate of OATP2B1. However, this effect has not been reported in humans.

Here's the gist: black tea contains plant compounds that, in lab dishes, can block a little transporter called OATP2B1 that helps pull certain drugs from your gut into your body. The worry is that tea might lower how much of those drugs you absorb. With levofloxacin, the concern would be getting a bit less of it into your system, which in theory could make it work less well against your infection.

That said, this is very theoretical. It comes from test-tube and animal studies (mostly with a cholesterol drug), and it hasn't actually been shown in people taking levofloxacin. A bigger, well-known issue with this antibiotic is that minerals (calcium, magnesium, iron) bind it up, so I'd separate it from milky or fortified drinks. If you enjoy tea, mention it to your pharmacist and they can help you space things out.

Black tea contains caffeine. Concomitant use of caffeine and quinolones can decrease caffeine clearance and increase effects and risk of adverse effects.

Here's the thing to know if you're sipping black tea while taking levofloxacin: the antibiotic can slow down how fast your body clears caffeine. So it's not the levofloxacin itself acting up, it's the caffeine in your tea hanging around longer and hitting harder than usual.

In practical terms, that cup of tea might leave you feeling more jittery, restless, or wide awake at night, with a faster heartbeat or a little extra anxiousness. The good news is this is fairly easy to manage. You don't need to quit your antibiotic. Just go easy on caffeinated drinks (tea, coffee, energy drinks, soda) while you're on levofloxacin, and pay attention to how you feel. If the buzz bothers you, switching to decaf or water for those few days usually does the trick. Always check with your pharmacist or doctor if you're unsure.

What Is Levofloxacin Used For?

Brand names: Levaquin, Levaquin Injection, Leva-pak

Levofloxacin treats bacterial infections in adults, including pneumonia (both community-acquired and hospital-acquired), skin infections, urinary tract infections, kidney infections, chronic bacterial prostatitis, and sinus infections. It is also used after exposure to inhalational anthrax and for plague.

Because levofloxacin carries significant risks, it should only be used for uncomplicated urinary tract infections, acute bacterial worsening of chronic bronchitis, and acute bacterial sinusitis when no other treatment option is available. Always use it only for infections confirmed or strongly suspected to be bacterial.
