PHENYTOIN (Dilantin)

Theoretically, guarana might reduce the effects of phenytoin and increase the risk for convulsions.
Guarana contains caffeine. Animal research shows that caffeine can decrease the anticonvulsant activity of phenytoin. The effect does not seem to be related to the seizure threshold-lowering effects of caffeine. However, the exact mechanism of this interaction is unclear.

Here's the gist for guarana and your phenytoin. Guarana is basically concentrated caffeine, and there's some thinking that it could chip away at how well phenytoin controls your seizures. In plain terms, that means a little more risk of a breakthrough seizure, which is the last thing we want. Phenytoin works directly to calm down overactive electrical activity in the brain, so anything that may weaken it deserves a second look.

The honest part: this comes from animal studies, and the exact reason isn't clear, so it's more of a theoretical caution than a proven problem. An occasional caffeinated drink likely isn't a big deal, but daily guarana supplements are a different story. Please loop in your doctor or pharmacist before adding it.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
