Dronedarone falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the thing to know about pairing guarana with dronedarone (Multaq): guarana is basically a concentrated source of caffeine, and dronedarone can mildly slow down the enzyme (CYP1A2) that your body uses to clear that caffeine. In plain terms, the caffeine may hang around a bit longer than usual, so you might feel more of the typical caffeine effects, like jitteriness, a racing or pounding heartbeat, trouble sleeping, or feeling on edge.

That last part matters here, since dronedarone is a heart rhythm medication and extra caffeine stimulation isn't ideal when you're managing atrial fibrillation. The good news is this interaction is rated minor and is only theoretical (based on lab data, not real patients). An occasional coffee is unlikely to be a problem, but I'd skip high-dose guarana energy supplements and check with your pharmacist or doctor first.

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
