Here is the interaction we found between these two, with the evidence behind it.
ANTIDIABETES DRUGS
Why this applies to Semaglutide
Semaglutide falls under ANTIDIABETES DRUGS — the group Guarana interacts with.
Some conflicting reports claim that caffeine, a constituent of guarana, might increase or decrease blood sugar.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture for your Ozempic, Wegovy, or Rybelsus and guarana question. Semaglutide isn't a prodrug, and guarana doesn't change how your body processes it. The concern is more indirect: guarana is basically a big dose of caffeine, and caffeine has mixed reports on blood sugar, sometimes nudging it up a little, sometimes down. So in theory it could make your numbers a touch harder to predict while you're working to control them.
That said, this is rated minor and unlikely, and the evidence is shaky. I wouldn't lose sleep over it. The bigger thing I'd watch is that caffeine can worsen the jittery, queasy, fast-heartbeat feeling some folks already get starting semaglutide. If you notice that, or your glucose readings look off, check in with your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Semaglutide page
“Semaglutide (Ozempic, Rybelsus, Wegovy) lowers blood sugar, and so do most of the 256 supplements on this page, which is why I read this list with real vigilance rather than a shrug. Nearly all of the entries either add to its glucose-lowering effect and raise the risk of hypoglycemia, or push sugar the other way and blunt it, as niaci…”

About Guarana
What is Guarana?
- Energy and alertness
- Reducing fatigue
- Weight loss support
- Athletic performance
- Mental focus
Guarana is an Amazonian seed that is naturally high in caffeine, which explains most of its stimulant and energy effects. While it may give a short-term boost in alertness and reduce fatigue, strong evidence for weight loss or major performance gains is limited, and it carries the same risks as too much caffeine. Talk to your pharmacist or doctor before using it, especially if you have heart problems, anxiety, or are pregnant.
About Semaglutide
What Is Semaglutide Used For?
Brand names: Ozempic, Rybelsus, Wegovy
Semaglutide is available as Wegovy (injection and tablet) and Ozempic (injection), and each product has its own approved uses. Wegovy injection is approved — alongside diet and exercise — to reduce excess body weight in adults and in children aged 12 and older with obesity, to lower the risk of serious heart events (heart attack, stroke, or cardiovascular death) in adults with obesity or overweight who also have established heart disease, and for treatment of a liver condition called MASH (metabolic dysfunction-associated steatohepatitis, formerly called NASH) with moderate-to-advanced scarring in adults. Wegovy tablets share the cardiovascular risk reduction and weight management uses in adults, but are not approved for MASH or for use in children. Ozempic injection is approved to improve blood sugar control in adults with type 2 diabetes (alongside diet and exercise) and to reduce the risk of major cardiovascular events in adults with type 2 diabetes who also have established heart disease.
All forms are used alongside a reduced-calorie diet and increased physical activity. Ozempic is not approved for type 1 diabetes. The MASH approval for Wegovy injection is an accelerated approval, meaning continued approval may depend on results from a confirmatory study.

Semaglutide Is Linked to Lower Levels of One Nutrient
Natural Medicines records Semaglutide as a medicine that can lower these. This is not an interaction with Guarana — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
