Mannitol falls under DIURETIC DRUGS — the group Guarana interacts with.

Theoretically, using guarana with diuretic drugs might increase the risk of hypokalemia.
Guarana contains caffeine. Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also lower potassium levels.

Here's the idea behind pairing guarana with mannitol. Mannitol is a strong osmotic diuretic, meaning it pulls extra fluid into your urine so you pee more. Along with that fluid, your body can lose minerals like potassium. Guarana is basically a concentrated source of caffeine, and large amounts of caffeine can nudge potassium out of your bloodstream too. Stack the two together and, in theory, your potassium could dip lower than it should.

Low potassium can leave you feeling weak, crampy, or give you an irregular heartbeat. To be honest, this is a theoretical concern based on how each works, not something proven in studies, and a little guarana in a drink is unlikely to matter much. Still, if you're on mannitol, check with your doctor or pharmacist before adding guarana.
