Levonorgestrel falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the key thing to understand about this pairing: it's really about the guarana, not the levonorgestrel. Guarana is loaded with caffeine, and levonorgestrel (like in Plan B or Mirena) can mildly slow down the liver enzyme, called CYP1A2, that normally clears caffeine out of your body. So in theory, caffeine could hang around a bit longer and feel a little stronger.

What that might look like for you is the usual too-much-caffeine stuff: jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious. The good news is this is rated minor and is only based on lab-type data, not real patient cases. It also won't make your levonorgestrel work any better or worse. If you're sensitive to caffeine, just go easy on guarana, and check with your pharmacist if you have questions.

Here's the thing to understand: this interaction runs in the opposite direction from what most people picture. It isn't that guarana changes how levonorgestrel works. Instead, contraceptive hormones can slow how fast your body clears caffeine, and since guarana is basically a concentrated caffeine source, that caffeine may stick around longer than usual.

The practical result is that you might feel the caffeine more strongly: a faster heartbeat, jitteriness, trouble sleeping, or a touch of anxiety. Keep in mind levonorgestrel as Plan B is usually a one-time dose, so this matters most with daily hormonal birth control like Mirena or pills. This is rated minor and is largely theoretical, so a normal cup of coffee likely won't be an issue, but heavy guarana doses could. If you're sensitive to caffeine, check with your pharmacist.

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
