Here's the gist when guarana shares your day with metformin: guarana is basically a concentrated caffeine source, and caffeine can nudge your blood sugar in either direction. Some studies show it bumping sugar up a little, others show a small dip. Because of that mixed picture, it could make your numbers slightly less predictable while you're working to keep them steady on metformin.

The good news is this is rated a minor and unlikely interaction, and the evidence behind it is fairly weak. An occasional caffeinated drink is rarely a problem. The thing to watch is regularly using strong guarana supplements, especially energy products. If you do, check your glucose a bit more often and have a quick chat with your pharmacist or doctor before making it a habit.

Guarana contains caffeine. Animal research shows that metformin can reduce caffeine metabolism. However, this effect has not been reported in humans.

Here's the gist: this interaction isn't really about your metformin getting stronger or weaker. It works the other way around. Guarana is basically a concentrated source of caffeine, and some early lab and animal studies suggest metformin may slow down how your body clears that caffeine. In theory, the caffeine could hang around longer and hit harder.

What might that feel like? The usual too-much-caffeine stuff: jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious. Keep in mind this has only been seen in animals, not people, so it's a pretty soft concern. Still, if you're sensitive to caffeine or have heart issues, it's worth a quick chat with your pharmacist or doctor before adding a guarana supplement.

Metformin is used to improve blood sugar (glucose) control in people with type 2 diabetes mellitus. It is always used together with diet and exercise, not instead of them.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
