Atazanavir falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here the spotlight is actually on the caffeine inside guarana, not on your HIV medicine itself. Atazanavir (Reyataz) can gently slow down CYP1A2, the liver enzyme that normally breaks caffeine down. When that pathway runs slower, caffeine can hang around longer than usual, so a typical guarana dose may hit you harder.

In plain terms, you might notice more jitteriness, a racing or pounding heartbeat, trouble sleeping, or feeling anxious or shaky. The good news: this is a theoretical, minor interaction based on lab data, not strong human studies, and it does not weaken how well atazanavir fights HIV. If you use guarana or energy products regularly, just mention it to your pharmacist or doctor so they can keep an eye on the caffeine load.

About Atazanavir

What Is Atazanavir Used For?

Brand name: Reyataz

Atazanavir capsules are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children 6 years and older who weigh at least 15 kg (about 33 lbs).

Reyataz — available as capsules and as an oral powder — is also approved for HIV-1 treatment in adults and in children as young as 3 months who weigh at least 5 kg (about 11 lbs). Reyataz oral powder must always be taken with ritonavir and food, and is not for use in children weighing less than 5 kg.
