Here's the gist for this pairing: Atazanavir is a strong blocker of a liver enzyme called CYP3A4, and that same enzyme helps clear modafinil, the active compound your body makes after taking adrafinil. So the concern runs toward adrafinil, not your HIV medicine. With CYP3A4 slowed down, modafinil could build up and hit harder, meaning more jitteriness, a racing heart, trouble sleeping, headache, or anxiety.

I want to be honest about the evidence here. This interaction is theoretical, based on how these drugs are processed rather than real-world reports. Still, atazanavir is a vital part of keeping HIV under control, and adrafinil is an unregulated supplement that's tough on the liver. Here's the concern with pairing these two. Once your body turns adrafinil into modafinil, that modafinil can mildly speed up a liver enzyme called CYP3A4. Your HIV medication, atazanavir (Reyataz), is broken down by that very same enzyme. So in theory, if it gets cleared a little faster, your atazanavir levels could dip lower than intended.

Why that matters: with HIV treatment, you want steady, reliable drug levels to keep the virus suppressed. Even small drops are a worry because they can risk the virus rebounding or becoming resistant over time. Keep in mind this is a theoretical interaction based on how the drugs work, not strong human studies, and modafinil's effect here is described as weak. Still, please run this by your HIV doctor or pharmacist before combining them.

About Atazanavir

What Is Atazanavir Used For?

Brand name: Reyataz

Atazanavir capsules (brands: Atazanavir, Atazanavir Sulfate, Reyataz) are used in combination with other antiretroviral medicines to treat HIV-1 infection in adults and in children 6 years and older who weigh at least 15 kg. It is not used alone — it is always part of a combination HIV regimen.

Reyataz oral powder is also approved for HIV-1 infection in adults and children 3 months and older weighing at least 5 kg, and must be taken with ritonavir. Neither atazanavir capsules nor Reyataz oral powder is recommended in infants under 3 months of age due to a risk of a serious brain condition called kernicterus.
