Here's the simple version: both guarana and nicotine are stimulants. Guarana is loaded with caffeine, and nicotine itself nudges your nervous system in the same direction. Stack them together and those effects can pile up. You might notice a racing or pounding heart, jitteriness, anxiety, trouble sleeping, or feeling a bit shaky or queasy. Nicotine isn't a prodrug here, so this is just two stimulants adding to each other, not one changing how the other is broken down.

To be honest, this concern is theoretical, based on how these substances work rather than strong studies. A normal cup of coffee probably won't be a big deal, but concentrated guarana supplements (energy products, pills) are where it adds up. If you're using a patch, gum, or lozenge to quit smoking and feel wired, check in with your pharmacist or doctor.

Guarana contains caffeine. Concomitant use of caffeine and nicotine has been shown to have additive cardiovascular effects, including increased heart rate and blood pressure. Blood pressure was increased by 10.8/12.4 mmHg when the agents were used concomitantly.

Here's the simple version: both guarana and nicotine are stimulants for your heart and blood vessels. Guarana is loaded with caffeine, and nicotine (whether from a patch, gum, lozenge, inhaler, or nasal spray) already nudges your heart rate and blood pressure up a bit. Put them together and those effects can stack on top of each other.

In one study, combining caffeine and nicotine pushed blood pressure up by around 11/12 points. You might notice a faster or pounding heartbeat, jitteriness, or feeling wired. This is based on how the two work rather than strong long-term data, so it's a real but modest concern, especially if you have high blood pressure, heart issues, or anxiety. If that's you, please check in with your pharmacist or doctor before pairing them.
