DIURETIC DRUGS
Why this applies to Methyclothiazide
Methyclothiazide falls under DIURETIC DRUGS — the group Guarana interacts with.
Guarana contains caffeine. Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also lower potassium levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with methyclothiazide and guarana. Methyclothiazide is a thiazide water pill, and one thing this type of medication tends to do is push potassium out of your body. Guarana is basically a concentrated source of caffeine, and in larger amounts caffeine can also nudge potassium lower. So the worry is that stacking the two could leave your potassium a little too low (called hypokalemia).
If potassium drops too far, you might notice muscle cramps, weakness, fatigue, or an irregular or fluttering heartbeat. I want to be honest though: this concern is mostly theoretical, based on how the two work, not on strong studies. An occasional small amount of caffeine likely won't matter much, but regular high-dose guarana could. Please run it by your pharmacist or doctor, who can also check your potassium with a simple blood test.

Methyclothiazide is used to treat high blood pressure. Methyclothiazide is also used to treat edema (fluid retention; excess fluid held in body tissues) caused by various medical problems, including heart, kidney, and liver disease and to treat edema caused by using certain medications including estrogen and corticosteroids. Methyclothiazide is in a class of medications called diuretics ('water pills'). It works by causing the kidneys to get rid of unneeded water and salt from the body into the urine.

Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems.
