Here the twist is that it's the guarana, not your conjugated estrogens (Premarin), that gets affected. Estrogen tends to slow down how your liver clears caffeine, and since guarana is basically a concentrated source of caffeine, the caffeine can hang around longer and build up. So your Premarin keeps working as usual, but you may feel the caffeine more strongly.

In practice that can mean the familiar too-much-caffeine feelings: jitteriness, a racing or pounding heart, trouble sleeping, anxiety, or an upset stomach. This is rated moderate and is mostly theoretical, so a little caffeine from coffee or tea is unlikely to be a problem. With a concentrated guarana supplement, though, it's worth chatting with your pharmacist or doctor first.
