Here's the thing about guarana: it's loaded with caffeine, and caffeine and theophylline are chemically close cousins. Your body uses the same pathway (mainly the liver enzyme CYP1A2) to clear both of them. So when you add a lot of caffeine from guarana, your liver can't break down theophylline quite as fast, and its levels can creep up. One report suggests clearance may drop by roughly a quarter.

Why that matters: theophylline has a narrow safety window, so even small jumps can tip you into side effects like a racing or pounding heart, jitteriness, trouble sleeping, nausea, or feeling shaky. This is based on how the drugs behave rather than big studies, but it's a sensible caution. Please check with your pharmacist or doctor before combining them.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
