Theoretically, concomitant use might increase the levels and adverse effects of both caffeine and riluzole.
Guarana contains caffeine. Caffeine and riluzole are both metabolized by cytochrome P450 1A2 (CYP1A2), and concomitant use might reduce the metabolism of one or both agents.

Here's the gist: both guarana's caffeine and your riluzole are cleared by the same liver enzyme, called CYP1A2. When they share the same exit door, they can slow each other down. That means caffeine may stick around longer (more jitteriness, a racing heart, trouble sleeping, anxiety), and riluzole levels could creep up too, which might add to side effects like dizziness, nausea, tiredness, or numbness.

Now, the honest part: this concern is theoretical, based on lab studies rather than real patients, so it may not amount to much for everyone. Still, an occasional energy drink is different from daily high-dose guarana pills. Please don't change your riluzole on your own. Just check with your pharmacist or doctor so we can keep an eye on how you feel.

What Is Riluzole Used For?

Brand names: Rilutek, Tiglutik, Exservan

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
